Treatment Instructions Form (pdf) - This may be printed and laminated to be posted for dental provider to reference for pre-treatment guidelines that ensure the best treatment outcome.
Treatment Consent Form (pdf) - Each patient must read and sign the consent form prior to treatment.
Post Treatment Instructions (pdf) - Give this form with post-treatment guidelines for best results to each patient.
To train your staff on this procedure:
- Print the Sinsational Smile Procedure Facts (Training Test) to review the facts. It may also be used as a test to ensure your team members are all on the same page.
- Watch the Sinsational Smile Training Video to have your staff see a video demonstration of this procedure.
- Review the Accelerator Light Manual included in your Sinsational Smile starter kit for cleaning and storage guidelines.
